Botox® for Chronic Migraine

What Is Botox® for Chronic Migraine?

Botox® (onabotulinumtoxinA) is the only FDA-approved injectable treatment for the prevention of chronic migraine in adults. Unlike pain medications taken after a migraine starts, Botox® works preventively — reducing how often migraines occur and how severe they are. It is administered every 12 weeks as a series of small injections around the head and neck.

How It Works

Botox® blocks the release of certain pain-signaling chemicals at nerve endings in the head and neck muscles thought to contribute to migraine. By reducing this chemical activity, the cascade that triggers a migraine attack is interrupted before it begins. Clinical trials showed that patients experienced an average of 8–9 fewer headache days per month after two treatment cycles.

What to Expect During the Procedure

•Preparation: No fasting or special prep is required. Arrive with a list of your current medications.

•Duration: Each treatment session takes approximately 15–20 minutes.

•Anesthesia: None required — injections are shallow and use a very fine needle; discomfort is typically minimal.

•Injection sites: 31 injections are placed at standardized sites across the forehead, temples, back of the head, neck, and upper shoulders.

•Same-day recovery: You can return to normal activities immediately after the appointment.

Conditions Treated

•Chronic migraine (15 or more headache days per month, at least 8 of which are migraines)

•Cervicogenic headache

•Tension-type headache associated with chronic migraine

•Medication overuse headache (in combination with a tapering plan)

•Occipital neuralgia-related headache patterns

Who Is a Good Candidate?

Adults who experience 15 or more headache days per month, have tried and not adequately responded to at least two oral preventive medications, and do not have a contraindication to botulinum toxin are strong candidates for this therapy.

Recovery & Results

Most patients do not experience significant downtime. Mild redness or tenderness at injection sites typically resolves within 24–48 hours. Full preventive benefit often builds gradually over the first two treatment cycles (approximately six months), so consistency with the every-12-week schedule is important. Many patients find headache days continue to decrease with ongoing treatment.

Frequently Asked Questions

How many treatments will I need before I notice a difference?

Some patients see improvement after the first treatment, but the best results typically emerge after the second or third cycle. Give the therapy at least two full cycles before evaluating effectiveness.

Can I still take my migraine rescue medication?

Yes. Botox® is a preventive treatment and is intended to be used alongside appropriate acute (rescue) therapies as directed by your provider.

Is Botox® for migraine the same as cosmetic Botox®?

The medication is the same, but the injection protocol, dosing, and sites differ significantly. Migraine treatment uses a standardized 31-injection pattern across the head and neck.
